REGISTRATION INSTRUCTIONS FOR
ALL CYS SERVICES PROGRAM
The Central Registration
office is located at the Child & Youth Services Program
at Building 847 behind the Chapel. The hours of
operation are 0800-1600. The phone number is
907-873-4599.
When you come in to
register your child/children, please bring the
following:
1. All forms from the packet, filled out
completely .
2. The Registration Fee
; $18 per child/youth not to exceed $40.00 per family.
3. Your child/children's shot records
(child shots must be current to Alaska standards and
must include a negative TB skin test)
4 . Proof of income ; ie. current
pay statements from both working parents, if applicable.

if you have any questions while filling out the
paperwork. Note that every blank must be filled in
before we can process the paperwork. The sponsor
should sign all paperwork if at all possible. At this
point children will either be given a spot in one of our
programs or be placed on the waitlist for care. |
